I know the use of affidavit for a birth certificate is currently, not very common since most agencies prefer a birth certificate.

What is an Affidavit of birth?
An affidavit of Birth is a legal document that contains facts that show/prove the circumstance of the birth of a certain person. It is used instead of a birth certificate. That means you don’t need this affidavit if you have a birth certificate.
This Affidavit is always sworn or affirmed by the third party as proof of the birth of another person.
That means you can’t swear or affirm facts adduced on your Affidavit of Birth but a person who knows important information about your birth can do.
It can be a father/mother/, grandfather/grandmother, brother/sister/, uncle/aunt, cousin, nephew/niece, or any near relative who was present at your birth or the doctor or midwife who delivered you.
The facts stated in the affidavit of birth are created under the penalty of perjury, so it must be truthful and accurate.
This document may be required in the passport application process, adoption, employment, pension funds, academic issues, etc.
An affidavit of birth must be notarized before you can use it as an official substitute for a birth certificate.
When do you need to use an affidavit of birth?
As I said earlier an affidavit of birth is an alternative document, there for you may use it only when;
- Your birth record is incomplete.
- You are unable to locate your complete birth record.
- You need additional proof of your birth when applying for a job or official documentation, such as a passport, visa, or Green Card.
Who can write an affidavit of birth
Anyone who is aware of the details surrounding a person’s birth may prepare an affidavit of birth on their behalf.
Technically is known as Affiant.
The affiant is the individual who drafts and swore the affidavit.
The person who requires a birth certificate frequently has a relative serve as the affiant.
The affiant need not be a citizen of the United States.
Affidavits for children are frequently written by parents.
How to write a valid affidavit of birth
Your birth affidavit must include specific information if you intend to use it for official purposes with organizations like the Department of State or the U.S. Citizenship and Immigration Services.
To ensure that your affidavit of birth satisfies the standards, be sure to include the following details.
- Applicant’s full name, place of birth, date of birth
- Full names of both of the applicant’s parents
- Statement of how the affiant knows of the applicant’s birth
- Full first name, middle name, and last name of each person whose name appears in the affidavit
- Mother’s name should be her current married name, any maiden name should follow in parenthesis
- Full dates, months, and years should be written out
Birth affidavit for passport application
You will need to supply a lot of information and documentation to prove your identity and citizenship if you are applying for a new U.S. passport at a U.S. passport agency using a DS-11 form.
Official proof of citizenship, either your birth certificate or paperwork proving your naturalization, is required for U.S. passport applications.
You will need to complete a DS 10 Birth Affidavit Passport Form if you are an application without a birth certificate.
An acceptance agent should get a DS-10 Birth Affidavit Passport Form along with any other necessary paperwork.
This comprises:
- a front and rear photocopy of the applicant’s driver’s license
- Census records, family Bible records, doctor’s records, Baptism certificates, and hospital certificates
The DS-10 Form calls for a written declaration from the responsible authorities stating that no birth certificate or record is in existence, as well as a photocopy of the applicant’s identification on both sides.
Anyone submitting a birth affidavit must be personally familiar with the details surrounding the applicant’s birth in the United States.
